1. The Accu-Chek Care Club membership/ID is not transferrable and is issued to an individual for their sole use.
    
2. By registering for Accu-Chek Care Club members agree to abide by the terms and conditions of the Accu-Chek Care Club.
    
3. 30 days’ notice will be given to Accu-Chek Care Club members should Roche Pakistan Limited and/or its affiliates decide to terminate the Program at any stage. Members will be notified on our website www.accu-chek.com.pk. From the date of such notice until the termination of the Program, members may redeem but not accrue points.
    
4. Roche Pakistan Limited and/or its affiliates may deduct/adjust the balance points in the customer's/member's account at any time and particularly those points that have been credited into the customer's/member's account in error by Roche Pakistan Limited and/or its affiliates.
    
5. Roche Pakistan Limited and/or its affiliates may make any changes to the terms and conditions and the reward plan at any time without any prior notice to the members of the Accu-Chek Care Club. Roche Pakistan Limited and/or its affiliates may take steps to inform the members of the said changes, however, any failure in doing so will not render Roche Pakistan Limited and/or its affiliates liable in any way.
    
6. Roche Pakistan Limited and/or its affiliates may collect information about members including information contained in the registration form and information as to transactions resulting in point’s credits and credits which will be held in the membership database. The membership database contains the member’s information such as name, email, address, telephone number, date of birth.
    
7. By registering for Accu-Chek Care Club and providing information to Roche Pakistan Limited, members hereby allow authorize permit and give consent to Roche Pakistan Limited or any other affiliate of the Roche Group to record, capture, save information in their customer databases or on third party server or cloud and use the same for internal purposes in the manner that Roche Pakistan Limited or any other affiliate of the Roche Group may deem fit.
    
8. The number of Accu-Chek Care Club Points earned within 12 months will be capped. Accu-Chek Care Club members can claim/redeem maximum 2 free products in a period of 12 months.
    
9. Roche Pakistan Limited and/or its affiliates reserve the right to suspend or terminate Accu-Chek Care Club Account, if Roche Pakistan Limited and/or its affiliates determine in sole discretion that Accu-Chek Care Club Terms and Conditions have been violated, or there is more than one Accu-Chek Care Club Account, or that the use of your Accu-Chek Care Club Account or participation in the Accu-Chek Care Club is unauthorized, deceptive, fraudulent. Roche Pakistan Limited and/or its affiliates may, in sole discretion, suspend, cancel, Accu-Chek Care Club Accounts that appear to be duplicative. In the event that if Accu-Chek Care Club membership is terminated, members will not be able to redeem any accumulated Accu-Chek Care Club Points.
    
10. Multiple entries received against the same ID/number within a short span or 1 month will be considered void.
     
11. Roche Pakistan Limited and/or its affiliates at any time and in our sole discretion, include our right to discontinue or change the Tiers (defined below) or benefits associated with it or adjust how Accu-Chek Care Club Points are earned, calculated or redeemed. It is possible that you may be unable to redeem Accu-Chek Care Club Points if Roche Pakistan Limited discontinues the Accu-Chek Care Club. If Roche Pakistan Limited and/or its affiliates make changes, we will post the amended Terms and Conditions to www.accu-chek.com.pk and update the “Effective Date”.
     
12. Employees of the Roche and/or Roche's affiliates, its retailers, its distributors, and members of immediate family of any such person(s) are not eligible to participate in the Accu-Chek Care Club. The term “immediate family” includes spouses, siblings, parents, children, grandparents, and grandchildren, whether as “in-laws,” or by current or past marriage(s), remarriage(s), adoption, co-habitation or other family extension, and any other persons residing at the same household whether or not related.
